Home visits project

Home health nursing can improve patient outcomes as well as have an impact on reducing the cost of healthcare. Timely visits by a home health nurse could help keep patients out of the hospital and reduce emergency room visits.

The purposes of home visits are to assess individual, family, and community health needs in the geriatric population. We will provide assessments, nonprescription medications as ordered by our partner physicians, health education, counseling, emotional support, medication management, minor wound care, and activities of daily living (ADLs).

Our field staff (qualified professional nurses and nursing assistants) will visit the less privileged elderly citizens at home, equipped with assessment tools, basic wound care supplies, durable medical equipment (ambulatory aide), and basic hygiene products. The field staff will carry backpacks full of the above-mentioned resources. We need help to increase field staff and maintain a consistent availability of resources.
